In the early 1980s, the industrial equipment supply chain was fragmented in exactly the way it still is for most plants today. Design firms handed off drawings they would never build. Fabricators welded steel they had not engineered. Lining contractors coated surfaces they did not understand. Every handoff was a failure point.
